| Description of Services: |
Supports are provided for people living with pre-diabetes or diabetes: one-on-one in person, virtual/ phone, or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices. In person consultation is offered in Chatham, Blenheim, Walpole, and Wallaceburg. The Diabetes Education Centre is accessed by a referral from physicians, nurse practitioners, midwives or through self-referral. Services and Programs: |
