| Description of Services: | Chatham-Kent Health Alliance’s (CKHA) Complex Continuing Care (CCC) Unit provides specialized inpatient care for medically complex patients, offering both short-term, goal-oriented care for those with potential for improvement, and long-term support for individuals with chronic conditions. The unit focuses on stabilizing patients and maximizing their functional abilities, with tailored therapy and medical oversight. For those with restorative potential, the aim is to facilitate a safe transition to appropriate community settings like assisted living or rest homes. For patients with long-term needs, the unit provides ongoing medical and nursing support. The CCC unit also serves as a transitional space for patients awaiting placement in alternate living arrangements or for low-intensity, “slow-stream” rehabilitation care for patients unable to tolerate high-intensity rehabilitation directly from acute care. | ||
