Organization: Windsor Essex Community Health Centre - weCHC
Description of Services: Primary Care: team of doctors and nurse practitioners offers healthcare to people of all ages living in Windsor and Essex County. Working with patients to keep them healthy and manage health concerns as they happen. This includes referrals to other weCHC programs.

Mental Health Care: short-term mental health counselling for those aged 12 and up, supporting clients with mild to moderate mental health issues, including mental illness, like anxiety and depression, stress, relationship, and family issues. This also includes AMANI - mental health and substance use support for Black youth

Addiction Support: addiction support workers provide counselling to those looking to make changes to their substance use and those who care for them. This includes access to harm reduction supplies.

Chronic Disease Care: team of professionals help clients better manage chronic disease and improve their overall wellness through education, respiratory care, therapeutic exercise and falls prevention.

Diabetes Wellness: Diabetes Wellness supports individuals 18+ diagnosed with Type 1, Type 2, Gestational, or Prediabetes, and those who care for them, manage their diabetes.

Hepatitis C Testing and Treatment: the Hepatitis C team helps prevent the spread of the Hepatitis C virus by providing our community with testing and treatment, medical management, counselling and education.

Teen Health: weCHC Teen Health offers medical and mental health support to youth aged 12-24, including counselling, addiction support, primary care and the Intensive Outpatient Program for Eating Disorders. Teen Health is also home to Black Health providing coordinated and collaborative care.
 
Identification Services - Health Cards Only Assists people without a fixed address (homeless, near homeless and people living in transient housing) in acquiring an Ontario Health Card
